Application for Gifted Education Universal Screening and Qualified Personnel Grant

Note:  The process below reflects the 2019-2020 application process.  Revisions to the 2020-2021 application process will be revised and posted by February 2020.

Universal Screening/Qualified Personnel Grant Directions and Application

Note:  There are three (3) required parts to complete the application.  All documents are scanned and then combined into one electronic document in PDF form that is emailed to the CDE Grants Department at CompetitiveGrants@cde.state.co.usNo faxes will be accepted.

Required documents included in combined electronic PDF submission to CDE Grants Department on April 15, 2019 include:

  1. Part IA: Complete Cover page (with signatures) and assurances page (Word)
  2. Part II:  Complete Part IIA and IIB (Excel)
  3. Supplemental Documentation
    1. Submitted with application when applying for universal screening:
      1. Vendors quote for acceptable testing costs
    2. Required documentation in DMS fiscal tab by grant submission date, April 15, 2019
      1. End of year expenditure report for grant account code: 3228. This report form is integrated into the gifted education fiscal tab in the Exceptional Student Services Unit, Database Management System (DMS), and is found at the end of the 2017-18 Expended Budget. (Only required if grant funds were received for 2017-18)
    3. Submitted with application when applying for qualified personnel:
      1. Evidence of degree or gifted education endorsement for qualified personnel
      2. Job description for qualified personnel (if not as yet hired)
      3. Salary schedule/letter for qualified personnel
    4. Required documentation in DMS fiscal tab by grant submission date, April 15, 2019
      1. When funds are used for qualified personnel working toward an endorsement, the AU must submit an attachment (on the DMS fiscal tab) providing evidence that personnel supported by grant funds is working toward an endorsement. This evidence may be in the form of a transcript of graduate credits, or documented professional learning hours aligned with gifted education core endorsement standards. (As per 2018-19 Exhibit A, Award Letter Statement of Work)
