Form to Request a Non-Public School Code

Please ensure  that your school meets the Legal Requirements for Non-Public Schools, including attendance law and health standards, you may complete the online form linked below to request a new code for a non-public school.

Home school programs may not receive CDE school codes. Please see the Home School Overview page or contact someone from the Schools of Choice unit if you have questions about non-public schools or home schools.

Complete the following information in order to assign your non-public school with a school code. Once the form is completed you will receive a copy of the submitted form. You will then be contacted via email by CDE staff with your newly assigned non-public school code.
