The introduction and questions below are meant to serve as only a starting point to help districts gather feedback from parents and teachers if they have not already done so.

Parent Survey
[DISTRICT] is committed to ensuring that everyone in our community – teachers, parents, families, and students – has input into how the education landscape looks as we begin the process of reopening schools. Please take a moment to take this brief survey, which will help [DISTRICT] better prepare and keep you engaged throughout this phase.
- When thinking about how you’d prefer to receive information from [DISTRICT], please indicate on a scale of 1 to 8 with 1 being the top preference and 8 being not preferred at all:
- Text messages

- Brief video highlighting major updates being considered as schools reopen

- How would you describe your level of comfort when it comes to sending your child(ren) to school this fall? Please mark the answer that best describes your feelings.
- If public health and school officials give the OK for school buildings to reopen, I will likely send my child(ren) if there are social distancing measures taken.
- If school buildings reopen, I will not send my child(ren) to school unless there is a vaccine or cure for COVID-19.
